Do I get Extra Help automatically?

You qualify for Extra Help automatically in three situations: you have full-benefit Medicaid coverage, your state helps pay your Part B premium through a Medicare Savings Program, or you receive Supplemental Security Income (Medicare.gov). If any of those apply, you do not need to do anything, and you will get a notice in the mail telling you what you will pay and which drug plan you have been enrolled in if you did not already have one.
The qualifier: everyone else has to apply, and this is where a very large number of eligible people fall through. If you are close to the income and resource limits but have none of the three automatic qualifiers, nothing happens until you file an application. Nobody contacts you. The program does not find you.

Two notes on the notices. Colored paper matters here: the letters Medicare sends about Extra Help arrive on yellow, green, purple, or orange paper, and they are worth keeping, because they are the proof your plan may ask for if it charges you the wrong amount. And if you get no notice at all before a new year, that usually means nothing has changed and you keep your Extra Help and your plan. Notices go out when you no longer qualify or when you are being moved to a different plan.

It is an official notice from Medicare about your Extra Help status, telling you what you will pay or that your Extra Help is changing. Keep it. If a pharmacy or plan later charges you more than your Extra Help level allows, that letter is the proof that fixes it.
If I lose Medicaid, do I lose Extra Help?
Not immediately. You keep Extra Help through December 31 of the year in which you qualified, even if your circumstances change mid-year. After that you would need to qualify on income and resources directly, which means applying through Social Security.
