With Extra Help in 2026 you pay no more than $5.10 for each generic drug and $12.65 for each brand-name drug, with a $0 plan premium and a $0 deductible (Medicare.gov). Once your total drug costs, including certain payments made on your behalf through the Extra Help program, reach $2,100, you pay $0 for each covered drug for the rest of the year.
The qualifier: these are ceilings rather than fixed prices, so you may pay less. And if you also have full Medicaid coverage and are in the Qualified Medicare Beneficiary program, your cost is lower still, at no more than $4.90 per covered drug. Insulin is separately capped at $35 for a month's supply for everyone with Part D, and Extra Help recipients often pay considerably less than that.
